A Lasting Power of Attorney (LPA) in Singapore is a legal document that allows a person (the "Donor") to appoint one or more trusted individuals (the "Donee(s)") to make decisions on their behalf if they lose mental capacity in the future. This can cover both personal welfare and property & affairs matters.

The LPA process in Singapore involves online application through the Office of the Public Guardian Online (OPGO), acceptance by the Donee(s), and certification by a designated Certificate Issuer.

What is an LPA? 

An LPA is a legal document where a Donor appoints Donees to make decisions on their behalf if they lose mental capacity. It can be for both personal welfare (like living arrangements and medical care) and property & affairs matters (like accessing bank accounts and managing property). The LPA only becomes effective when the Donor loses the ability to make their own decisions. 

Who can make an LPA? 

The Donor must be at least 21 years of age and have the mental capacity to make the decision. The Donor must not be an undischarged bankrupt if the LPA covers property and affairs. 

Making an LPA

 1. Online Application: The Donor must complete the LPA application online through the Office of the Public Guardian Online (OPGO).

2. Donee(s) Acceptance: The Donee(s) must log in to OPGO and accept their appointment. 

3. Certification: The LPA must be certified by a Certificate Issuer (accredited medical practitioner, practicing lawyer, or psychiatrist). 

4. Fees: There are application fees (waived for Singapore citizens until March 31, 2026) and certificate issuer fees.

What is an LPA?

Who may not be suitable for LPA? 

A person who lacks mental capacity will not be a suitable candidate to do an LPA. A lack of mental capacity could happen due to:

  • - advanced dementia
  • - a mental illness

How to Apply for an LPA Online in Singapore

The process of applying for an LPA in Singapore involves three steps:

  • Step (1) Complete the LPA Form Online 
    • Depending on your preferences, there are two types of LPA forms:
    • - LPA Form 1: This default form grants your donee general powers with some basic restrictions to make decisions on your behalf.
    • - LPA Form 2: If you want to specify the exact scope of your donee's power, consult a lawyer to draft a formal document.
    •  Both forms can be filled out Online (Click Here) on MSF Office of the Public Guardian (OPGO) Website. Complete your chosen form and ask your appointed donees to accept their nomination.
  • Step (2) Certification by an LPA Certificate Issuer 
    • Your LPA must be witnessed and certified by an LPA Certificate Issuer. 
    • In Singapore, an LPA can be issued by accredited medical practitioners, practicing lawyers, or registered psychiatrists. 
    • The certification session typically takes about 25-30 minutes, and your appointed donees don't need to be present at the clinic.
  • Step (3) Registration by the Office of the Public Guardian (OPG) 
    • After your LPA is certified at an LPA clinic, you'll need to self-apply for official registration with the Office of the Public Guardian (OPG).
    • You will receive an SMS or email notification once your LPA is officially registered.

LPA Form 1 is the standard form used by most people to make an LPA. You can prepare an LPA Form 1 online on OPGO Website by yourself. It allows you to: 

  • appoint up to 2 Donees 
  • appoint 1 Replacement Donee (optional) 
  • grant your Donees powers to decide on your personal welfare, property and affairs, or both 

LPA Form 2 is the non-standard form to make an LPA. You must consult a lawyer to prepare an LPA Form 2.  The lawyer will do this online, and you'll be asked to review and accept your LPA draft online. This is for those who want to: 

  • appoint more than 2 Donees 
  • appoint more than 1 Replacement Donee 
  • grant your Donees specific or customised powers 

There are 2 fees invovled:
1) The LPA certification fee is paid to the certificate issuer.
2) The LPA Application fee is paid to the OPGO

You need the Singpass app to digitally sign on your LPA during your visit to the LPA certificate issuer.

After your LPA has been registered, you and your Donees can retrieve it at any time by logging in to the Office of the Public Guardian Online (OPGO) with your Singpass.

If you would like to keep a printed copy of the LPA for personal reference, you can print the PDF after downloading it from OPGO. However, the printed copy will be watermarked and cannot be used for transactions.

Your Donee will only be able to make decisions for you if you lose mental capacity. This must be certified by a registered medical practitioner. If you lose mental capacity, your Donee can make decisions for you by transacting directly with public and private organisations.

When doing so, they will need to produce: your registered LPA (by sharing it via Office of the Public Guardian Online) the medical report on your loss of mental capacity other supporting documents as required by the organisation..

Advanced Medical Directive

You do not need a lawyer to make an AMD. To make an AMD, you will have to consult a doctor who, as a witness, is required to explain the AMD. You may have to pay the doctor for the services rendered. However, the AMD form itself is free.

You can fill in your details on the Advance Medical Directive (AMD) Form 1 first. However, the two witnesses (one of whom needs to be a Doctor) and you must sign the form at the same sitting.

If you have made an AMD, regardless of objections from members of your family, the doctors will have to respect your AMD. Hence, while making an AMD is a very personal decision, we encourage you to discuss it with your family members and loved ones. This will help them to understand and respect your wishes should you become terminally ill.
